Required Registration Documents

Important Reminders:

  • Only parents, legal custodians and/or legal guardians can register students in the district.
  • All documents required can be uploaded during the online registration process OR brought to the Registration Department Office located at 7377 Chateau Drive in Parma. Please call (440) 885-8796 to schedule a time to bring them in. The Registration Office is open from 7:30 a.m. to 3:00 p.m. 

You will need to have the following documents:

  • Student's birth certificate showing birth parent(s) names, date of birth and name of student OR a Passport (nothing can be blacked out or retracted)
  • Student's immunization record

You will need to have the following documents:

  • Parent/Guardian Photo ID
  • Student's immunization record
  • Two Proofs of Residency documents (a Primary and a Secondary). Must not be older than 30 days: 
    • ONE PRIMARY PROOF OF RESIDENCY MUST BE: A SIGNED LEASE, A MORTGAGE STATEMENT, DEED OR AN OWNER -TENANT AFFIDAVIT (this form can only be signed in-person in our Registration Office and notarized by us). 
    • Second Proof of Residency acceptable documents: utility bills (or a letter of verification of utility service for water, gas, or electric: we do not accept utility bill Termination Notices), official purchase agreement, tax statements, pay stub, County Tax Bill, W2, homeowners, renters or car insurance bill or statement, official government correspondence 
    • Please note: we will NOT accept: cell phone bills, bank statements, or general letters from Job and Family Services.
    • **IF YOU DO NOT HAVE A PRIMARY PROOF OF RESIDENCY LISTED ABOVE, WE WILL NOT BE ABLE TO ENROLL YOUR STUDENT
  • Certified verification of legal custody/guardianship (if applicable) - If the child is not living with both biological parents, provide a certified copy of the Divorce Decree/Dissolution AND Shared Parenting Plan, if applicable; or provide a certified Judgment Entry showing custody.  A certified copy has a stamp showing it has been filed with the court and is signed by a judge. ORC 3313.672

  • Individualized Education Program (IEP) or Evaluation Team Report (ETR) or 504 (If applicable)

If there is no court-ordered custody and the parents were never married, the mother is considered the sole residential parent and legal custodian pursuant to O.R.C. 3109.042(A) and the child must be enrolled in the school district in the city where the birth mother lives – until a court determines otherwise. As an alternative, the other parent may seek to obtain legal custody through the courts. If he/she does, the child may be admitted tuition-free, for up to sixty days pursuant to O.R.C. 3313.64(E). For the District to consider a sixty-day provisional enrollment, the adult must provide evidence of application for legal proceedings or a notarized sworn statement that the adult is seeking custody of the child through the court system.
