List of Required Registration Documents

Important Reminders:

• Only parents, legal custodians and/or legal guardians can register students in the district.
• All documents required can be uploaded during the online registration process OR brought to the Registration Department Office located at 7377 Chateau Drive in Parma. Please call (440) 885-8796 to schedule a time to bring them in. The Registration Office is open from 7:30 a.m. to 3:00 p.m. 

You will need to have the following documents:

• Student's birth certificate showing birth parent(s) names, date of birth and name of student OR a Passport (nothing can be blacked out or retracted) or Birth Affidavit
• Student's immunization record
• Two Proofs of Residency documents (a Primary and a Secondary). Must not be older than 30 days: 
  • ONE PRIMARY PROOF OF RESIDENCY MUST BE: A SIGNED LEASE, A MORTGAGE STATEMENT, DEED OR AN OWNER -TENANT AFFIDAVIT (this form can ONLY be signed in-person in our Registration Office and notarized by us).
  • Second Proof of Residency acceptable documents: include: utility bills (or a letter of verification of utility service for water, gas, or electric--we do not accept utility bill Termination Notices), official purchase agreement, tax statements, County Tax Bill, W2, homeowners, renters or car insurance bill or statement, official government correspondence 
  • Please note: we will NOT accept: cell phone bills, bank statements, pay stubs or general letters from Job and Family Services.
  • **IF YOU DO NOT HAVE A PRIMARY PROOF OF RESIDENCY LISTED ABOVE, WE WILL NOT BE ABLE TO ENROLL YOUR STUDENT
• Parent/Guardian Photo ID

• Certified verification of legal custody/guardianship - If the child is not living with both biological parents, provide a certified copy of the Divorce Decree/Dissolution AND Shared Parenting Plan, if applicable; or provide a certified Judgment Entry showing custody.  A certified copy has a stamp showing it has been filed with the court and is signed by a judge. ORC 3313.672

• Individualized Education Program (IEP) or Evaluation Team Report (ETR) or 504 (If applicable)

If there is no court-ordered custody and the parents were never married, the mother is considered the sole residential parent and legal custodian pursuant to O.R.C. 3109.042(A) and the child must be enrolled in the school district in the city where the birth mother lives – until a court determines otherwise. As an alternative, the other parent may seek to obtain legal custody through the courts. If he/she does, the child may be admitted tuition-free, for up to sixty days pursuant to O.R.C. 3313.64(E). For the District to consider a sixty-day provisional enrollment, the adult must provide evidence of application for legal proceedings or a notarized sworn statement that the adult is seeking custody of the child through the court system.

First Step Preschool

First Step Preschool registration is a two step process:

Step 1: Please contact First Step Directly at (440) 885-8645 to fill out a Peer Model Application. After this application is complete and received by First Step, move on to Step 2. 

Step 2: Complete the online Enrollment Application using the RED button above.

More information about School of Choice

February 2026 update:

After a thorough review of staffing and scheduling considerations, we have determined that the financial impact at the high school level can be mitigated due to the flexibility of student schedules. Because high school students have a variety of course schedules within the schools’ schedule, the staffing implications are not as significant. As a result, we will continue offering School of Choice at the high school level. 

School of Choice (SOC) options at elementary and middle schools will be phased out. Any previously approved SOC students will remain at their school of choice until it is time to move on to a different school, based on their grade, e.g., elementary to middle school. So a student who was approved for school of choice in an elementary building can stay in that building until it is time to move to middle school. The same is true for SOC middle school students; they can stay at their school of choice until it is time to move to high school. Additionally, for next year, we will allow siblings to apply to attend the same school as their sibling who was already enrolled in their school of choice. 

For the upcoming 2026-2027 school year:

• Current SOC students may remain at their school without reapplying. 
• Once approved, students may continue at that school for the remainder of their time in that building. For example, they will stay at their elementary school until transitioning to middle school or they will stay in their middle school until it is time to go to high school. 
• Siblings of current SOC students may apply for SOC at the building where their sibling attends.
• Applications for School of Choice for high school students are required. Course availability varies, and specific course requests may not be available. 
• Applications for School of Choice for any elementary or middle school students who are siblings of current SOC students are required. Acceptance is based on a sibling attending that school.  
• School of Choice is an option that can be revoked if student attendance or behavior is a concern.  
• School of Choice may be denied based on capacity limits by grade level, school building, and educational program.
• District Bus transportation is not provided for school of choice students. 
• School of Choice conditions do not apply to students who attend a different school based on special education services. 
• The SOC application will open on March 2, 2026, and remain open until March 26, 2026.
