State of Ohio Meningococcal Vaccine requirements for students entering seventh and 12th grades:

• 6th Grade Tdap Booster and Meningococcal Vaccine:

  All 6th grade students going into 7th grade must have their Tdap Booster and Meningococcal Vaccine on file with the clinic by the start of the 2024 - 2025 school year.  Please make arrangements over the summer to get these immunizations, complete this paperwork, and make sure that it is turned into your child's school clinic prior to the start of school in August.  Students that don’t have paperwork on file by the 15th day of school may be excluded per Section 3313.671 of Ohio Revised Code.  If you have any questions please feel free to contact your child's clinic.

   

• 11th Grade Meningococcal Vaccine:
