Medications at School

    The Parma City School District has adopted the following policies in regard to medications at school:

    • A signed permission form with all sections filled out completely by parent/guardian and a physician.

     

    • All medications must be in the original bottle with the current label.

     

    • If there are any changes with the medication, dosage or time to be given a new medication form must be filled out completely and returned to school.
