Immunizations

Students enrolled in Preschool through grade 12 must have written proof of immunizations currently required under Ohio law on file at their school. Ohio law requires schools to exclude students from attendance no later than 15 school days after admission if they are not properly immunized. Remember to update the clinic with any new immunizations received. Please send a copy of the updated record that includes month, day and year of immunization. Your School Health Specialists can answer any questions regarding what you will need to fulfill your requirements for immunizations.
PLEASE UPDATE YOUR SCHOOL AS SOON AS POSSIBLE IF YOU HAVE ANY NEW VACCINES GIVEN.
Ohio Department of Health requires preschool students to have a physical and oral assessment done yearly and be on file at the school within 30 days of enrollment.
